Seasons Hospice Foundation’s Memorials Program offers several different options so you can best reflect the life of your loved one and fulfill your personal intentions.
SEASONS HOSPICE FOUNDATION MEMORIAL & HONOR GIVING
As caregivers, family, friends and colleagues, the Seasons Hospice Foundation understands your desire to celebrate and honor the legacy of your loved one.
We recognize that memorial giving is an expression of love, so we have developed the Memories Are Timeless program. With this memorial program, you have the opportunity to honor a special person or make a memorial donation that will help make a difference in the lives of terminally ill children, adults, and their families around the country.
A gift or memorial donation is a fitting tribute to someone special. Your support helps Seasons Hospice Foundation provide a wide range of programs such as fulfilling wishes to terminally ill patients and their families, children’s bereavement camps, children’s bereavement libraries, legacy projects, community education and applied research – and so much more.
You can honor a special someone, an AccentCare Hospice employee or a volunteer in recognition of their outstanding achievement and/or service. You can also honor an occasion such as a wedding, bar/bat mitzvah, birthday, etc.

MEMORIAL GIVING
Make a donation to Seasons Hospice Foundation in memory of your loved one. The Foundation will acknowledge your donation and notify the caregiver and/or person of your choice that you have made a gift. (Your gift amount will stay confidential).
IN HONOR
If you are honoring an employee, person or occasion, the foundation will acknowledge your donation and notify the special person that you have made a gift in honor of them and the occasion if applicable. (Your gift amount will stay confidential).
A SEASON OF REMEMBRANCE
Seasons Hospice Foundation has two programs for families to create a lasting legacy to honor the memory of their loved one. A Tree of Remembrance resides at Swedish Covenant Hospital in Illinois. This is a place where family and friends can remember their loved ones or pay tribute to a special someone by placing their name or description on a leaf and add it our tree. Our hospice inpatient centers in Naperville, Illinois and Milton Massachusetts house our interactive memorial displays capable of memorializing your loved one through photos and storytelling.

Whether you choose a leaf, the tree trunk, dove or our interactive display, your honoree’s name will be held in a place of tribute in our AccentCare Hospice Inpatient Unit and you can be confident that your generous gift will have a meaningful impact on the lives of others. MEMORIAL GARDENS
Both the Hospice Inpatient Center in Milton, Massachusetts and in Naperville, Illinois include beautiful Memorial Gardens as part of their serene environment. These gardens feature lovely pathways consisting of custom engraved bricks. We invite you to pay tribute to your loved one with an engraved brick in their honor. Different sized engraved bricks are available. The message on each brick can be personalized. Just complete the appropriate form and send it to to Foundation@seasons.org
